The file format is .doc, written with Word 2003. I don't convert it to .docx and deliver it back in .doc.
Helpful may be the post from Dragon-User, 13.04.2010. Also for reasons of time I correct a lot of small syntactic or spelling errors manually.

Wrong again then , and I would agree with your assumption that it is related to the continual corrections and causing Dragon problems in tracking the select and say text.
